The second grant shared by Freed Center for Independent living, Community Recovery Resources and Western Sierra Medical clinic will provide for navigation of patients through all three systems to provide the best combination of services for a particular individual, with the goal of reducing hospital readmission rates. For instance, Freed can work with seniors and people with disabilities, CoRR can provide substance abuse programs and support, and Western Sierra medical Clinic provides medical dental and behavioral health care to people regardless of their ability to pay.
Dignity Health grants have supported 3,000 projects and programs totaling nearly $47 million dollars since the grants began in 1990.
